Problem Statement

  • Drivers have to fill 6 forms- Authorization, Ackowledgement, Receipt of Drug and Alcohol Policy, Background Investigation, FMCSA D&A, Limited Consent for business
  • Drivers who have to review and acknowledge the forms have to be notified to complete the
  • Completed forms needs to be consolidated as a PDF document and sent to third party for validation and further processing


  • Drivers who need to fill the forms are identified and notified through mobile notifications to complete the forms
  • Nintex forms are developed where drivers fill in their primary information like FLID and SSN in the mobile
  • After the entered information is validated , form is prefilled with driver’s information from
  • Tasks are assigned to the drivers to review and sign the
  • For each form a task is assigned , for the drivers to review the form and
  • When they complete each form, PDF is generated for each form , which contains the completed form of the driver with the date and
  • Task Reminders are sent to drivers ,if they haven’t completed the
  • After completion of forms, the signed forms are combined as PDF and sent to third party for business needs
  • Reports of drivers who did not complete the forms are captured and sent to leadership.


  • The process automation improved the productivity and efficiency atleast by 25% and reduced manual follow up and tracking
